Case Management Along the Continuum |
September 7-10,
2008
San Francisco, CA
Highlights and Benefits
The evolving roles of case management are being shaped by changing
legislation and regulatory guidelines regardless of your health care
work setting. Key challenges that we must address include providing
patient continuity of care; admitting patients to the appropriate level
of care, ensuring that documentation reflects evidence-based practice;
communicating effectively across settings; addressing the needs of the
uninsured, and many more. How can we address these issues and optimize
functional case management practices within our health care industry?
Join us at our 14th annual Case Management Along the Continuum
conference and fine-tune your practice to be the best that you can be.
Learn what is working in other institutions and network with colleagues
who are challenged with the same issues. Meet those who are creating
strategies to respond to evolving needs.
